Student-athletes must have a Pre-Participation Packet completed and on file in the Athletic Training Room prior to working out with an athletic team.  This applies for any student athlete in grades 7-12. 

Pre-participation packets can be obtained from the athletic training office, front office or can be downloaded from this site (see link on left-side of page). 

Please turn completed packets directly into the athletic trainers on your campus.  This will prevent any forms being misplaced.
